Request approval to hire before you advertise: the position, new or replacement, the reason, hours, pay range, start date, budget and sign-off lines. Print it or download text and CSV, without signing up.
APPROVE THE HIRE FIRSTNo account needed
Edit the fictional request, or start blank. Your requisition stays in this tab if you refresh; download or print what you want to keep. Nothing is uploaded or sent, and this form does not approve the hire or post a job ad.
Step 1 of 5The request
Step 2 of 5The position
Say what work is not getting done, or what cover is missing, and what changes if the role stays empty.
Step 3 of 5Hours and pay
Pay rules differ by place. Some US states and cities require a pay range in job ads, and the range you agree here often goes into the ad. Check the rules where you hire; this is not legal advice. Sources are linked below the tool.
Step 4 of 5Skills
List what the job needs on day one. Training you will give belongs in the job ad, not the requirements.
Step 5 of 5Sign-off
Add a line for each person who signs the request off, in the order they sign. Leave a name blank to write it in by hand.
Sign-off 1
This is already the first sign-off line.

Sign-off 3
This is already the last sign-off line.
Before you send it
Every detail approvers usually ask about is filled in.

Internal request only: not a job ad, job offer or contract. Pay and hiring rules, including pay ranges in job ads, vary by place. This is not legal advice.
Ask for approval to hire before anyone writes the job ad. This free job requisition form records who is asking, the position, whether it is a new role or a replacement, why it is needed, the hours, the proposed pay range, the target start date and the budget, with sign-off lines for each person who approves it. Start with the fictional sample, replace the details and print the form or download it as text or CSV.
What is a job requisition?
A job requisition is an internal request to fill a position. A manager fills it in, and the owner or whoever controls the budget approves it before the vacancy is advertised. It is not the job ad: the requisition explains to your own business why the role is needed and what it will cost, while the job ad maker turns the approved details into a public advert and a Now hiring sign.
Even in a small shop, a short written request helps. It makes everyone agree on the hours and pay before a candidate hears them, and it leaves a record of why the role was opened.
Make a job requisition form
Enter the request: a title for your files, an optional requisition number, the request date and the requesting manager, with their department and location.
Describe the position: the job title, who it reports to and how many openings there are.
Choose New position or Replacement. For a replacement, name the person who is leaving and their last day, if you know it.
Explain the reason for the request in a few sentences: the work that is not getting done, or the cover that is missing.
Set the employment type, the hours a week and the usual schedule, the proposed pay range and the target start date. Seasonal and temporary roles can have an end date.
Add budget notes, the required skills (one per line) and any nice-to-have skills.
List the people who sign it off, such as the requesting manager, the owner and payroll. Each gets a line for a signature, a date and Approved or Not approved.
The draft stays in this browser tab until you close it. Print it, save it as a PDF, or download a text or CSV copy for your records.
Write a request that gets a clear answer
Lead with the reason. "Weekend cover falls to two full-time staff who already work five days" gives the approver something to weigh; "we are short-staffed" does not. If the role replaces someone, say whether the hours and pay stay the same or change, since that is usually the first question.
Keep the skills list to what the job needs on day one, such as taking card and cash payments or lifting delivery boxes. Put training you will provide and things that would be nice to have in the separate list, so the job ad that follows does not ask for more than the role requires.
Give a pay range you could actually offer. The form estimates the yearly base pay for each position from the range: an hourly rate × the hours a week × 52, a weekly rate × 52 or a monthly rate × 12. In the sample, $16.50 to $18 an hour for 16 hours a week comes to $13,728 to $14,976 a year. That is base pay only, before employer taxes, benefits and paid leave, so use the staff cost calculator for the full weekly and yearly cost.
Pay ranges and hiring rules
Rules about recruiting, pay and job adverts differ from place to place. Some US states require a pay range in job postings, often depending on how many people you employ, for example New York, which explains its pay transparency law, and Washington, under its Equal Pay and Opportunities Act. The EEOC describes prohibited practices in recruitment and hiring under US federal law. In the UK, Acas notes that there are no set recruitment processes required by law but employers must follow a fair process, and its recruitment guidance covers each stage.
The requisition itself stays inside your business, but the pay range you agree here often goes into the job ad. Check the rules where you hire. This page is not legal advice.
Print and get it signed
The preview is the printout. Choose Print / PDF to print it on Letter or A4 or save a PDF to email to the approvers. Each sign-off line prints with space for a signature and date and boxes for Approved or Not approved. The form does not send the request to anyone or record a decision.
Once the request is approved, write the advert with the job ad maker and plan the interviews with the interview scorecard, using the required skills as the questions to ask about.
Frequently asked questions
Can I make a job requisition form without an account?
Yes. The form opens with an editable sample and works without a login or email address. Nothing you type is uploaded.
What is the difference between a job requisition and a job ad?
A requisition is the internal request to hire, with the reason, budget and approvals. A job ad is the public advert applicants read. Most businesses approve the requisition first, then write the ad from it.
Does the form approve the hire or send it to anyone?
No. It prepares a document. The approvers sign the printed copy or reply to the PDF through your usual process.
Does the page keep my requisition?
The draft is held only in this browser tab: it survives a refresh, and closing the tab clears it, so download or print the form before you leave.
